What is Signature Aviation’s Wellness Incentive Program?
This is a voluntary wellness program made available by Signature Aviation to team members enrolled in the Company’s U.S. medical plans through the Aon Active Health Exchange. If you choose to participate in the program you will be given an opportunity to complete the following voluntary activity to earn a wellness credit on your paycheck.
- Complete a Biometric Health Screening and submit the results of the screening to Actuarial Consulting Services, Inc. (ACS), the vendor managing our Wellness Incentive Program.
To receive the credit, you and your spouse (if enrolled) are required to complete this step. After completing the steps, you will earn a wellness credit that will appear on your paycheck.
The program does not apply to employees working in Hawaii and San Francisco.
Please review an important notification for you and your spouse about the collection, uses and disclosures of your health information. This notice is found on the Benefits Portal at http://yoursignaturebenefits.com. Click on the ‘Tools and Resources’ tile and select ‘Wellness Incentive Program’.
How does it work?
Team members enrolled in medical coverage through the Aon Active Health Exchange will have an opportunity to earn a wellness credit by completing a biometric health screening. Once the credit has been earned and processed, the wellness program vendor, ACS will notify Signature Aviation and you will start to see the wellness credit appear on your paycheck in 2 to 4 weeks or as soon as administratively possible. The credit will not be applied retroactively. The sooner you complete the activities the quicker you will earn your credit. Your credit is good for the remainder of the current year and will rollover into the next plan year. [For example, if you earned your wellness credit and the biometric screening took place in the calendar year 2020, your credit will be valid for the remainder of the current year, plus the following 2021 plan year. In 2021, you will need to complete the steps again so it can be applied to the 2022 plan year and so forth.]
How much will I save?
The wellness credit will appear as an earning on your bi-weekly paycheck in the amount of $18.46 minus taxes. The savings adds up and has the potential to be worth approximately $480 annually.
Will my dependents need to complete the process?
If your eligible spouse is enrolled in the Signature Aviation medical plan, he/she is required to complete the same steps in order for the wellness credit to apply. If only one of you completes the steps, the credit will not be awarded. The program requirements do not apply to dependent children.
How do I get started?
You and your spouse (if applicable) will need to complete a Biometric Health Screening. Follow the steps below:
- Download & print the Wellness Screening Form found at www.yoursignaturewellness.com or the benefits portal at http://yoursignaturebenefits.com.
- Make an appointment with your doctor or healthcare clinic.
- Take the Wellness Screening Form to your appointment.
- Ask your doctor to complete, sign and give the form back to you.
- Submit your completed Wellness Screening Form directly to the Signature Aviation Wellness Program.
What is a Biometric Health Screening?
A biometric health screening is the measurement of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, and blood glucose. Your doctor will collect this information during a wellness exam or physical, also known as preventive health services. Afterwards, your doctor will record your measurements on the Wellness Screening Form.
What is the Wellness Screening Form?
The form collects information from your biometric health screening. The Wellness Screening Form can be found by logging onto: www.yoursignaturewellness.com or by visiting the benefits portal at http://yoursignaturebenefits.com Print out the form and give it to your doctor or healthcare provider to complete and return back to you.
Can I earn the discount if I choose a different medical carrier in 2021?
Yes. The Wellness Incentive Program allows you to earn and receive your credit if enrolled in a Company plan, regardless of which medical insurance carrier you choose within the Aon Active Health Exchange. However, it does exclude plans and carriers in Hawaii and San Francisco.
Is there a cost for Preventive Health Coverage?
Preventive biometric health screenings for most medical plans are covered at 100% and are not subject to a deductible or co-pay if your doctor or clinic is in-network. There may be exceptions based on your individual circumstances. Check with your medical insurance carrier if you have questions about costs.
What are Convenience Care Clinics?
If you have difficulty scheduling an appointment with your doctor, you may want to choose to have your biometric screening performed at a convenience care clinic such as CVS Minute Clinics or Walgreens Take Care Clinics. Check with your medical insurance carrier for a list of in-network convenience care clinics.
Can I use lab results from a previous doctor visit?
If you or your spouse visited a physician earlier this year and completed lab-work, you can request that your doctor complete the Wellness Screening Form using that information for your biometric health screening.
Is there a deadline for the Wellness Incentive Program?
You can earn your discount at any time during the year. As soon as you and your spouse (if enrolled) submit the completed Wellness Screening Form to the Signature Wellness Program either online or by mail, you will see your credit appear as soon as administratively possible. Please allow 2 to 4 weeks for the wellness credit to be reflected on your paycheck. The credit will not be applied retroactively. The sooner you complete the activity the quicker you will earn your credit.
- To guarantee that your wellness credit appears on the first paycheck of 2021, you (and your spouse if applicable) must submit the completed wellness screening form to ACS by November 20, 2020.
- Remember! If you already earned your wellness credit this year and your biometric screening results were performed in 2020, you will automatically receive the wellness credit for 2021. If you are not sure when you earned your credit, contact the Signature Aviation Wellness Program at www.yoursignaturewellness.com or the Signature Benefits Team at firstname.lastname@example.org.
- Keep in mind if you are enrolling a spouse for the first time during the 2021 enrollment period, they are also required to complete the steps before the credit can be awarded.
If you miss the November 20, 2020 deadline, it’s okay! You and your spouse (if enrolled) can still earn the credit. Just complete the activity and submit the wellness screening form as soon as possible to start saving money. The credit will not be applied retroactively and may take 2-4 weeks or as soon as administratively possible to appear on your paycheck.
What if I don’t see the wellness credit reflected on my paystub?
Your credit will appear as income on your pay stub. If you do not see this, verify that you completed your Wellness Screening Form and submitted it to www.yoursignaturewellness.com or mailed it to Signature Aviation Wellness, P.O. Box 390841, Omaha, NE 68139. You can confirm that your form was received and accepted by contacting the wellness program vendor, ACS, online at www.yoursignaturewellness.com. Keep in mind it takes 2-4 weeks for processing and you should receive your wellness credit as soon as administratively possible. What if I have a mid-year change to my benefits?
The wellness credit is tied to your medical election. If your medical coverage ends, you will no longer receive a wellness credit. If you have a qualified life event in which you dropped medical coverage and enroll again during the year, the wellness credit you previously earned for 2021 will be applied. If your spouse is added onto your medical plan mid-year because of a qualified life event, it will not impact the credit you earned for the year. Keep in mind, your spouse will need to complete the required steps if you enroll them on your medical plan in the following year.
Where can I find out more information about the Wellness Incentive Program?
Signature Aviation makes available an important notification about the Wellness Incentive Program, including details about the collection, uses and disclosures of your health information. The Program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health and prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act of 1996, as applicable, among others. The notice can be found at www.yoursignaturewellness.com or on the benefits portal at: www.yoursignaturebenefits.com. From the Home page, click on the ‘Tools and Resources’ tile and select the ‘Wellness Incentive Program Notification’.
The Signature Aviation Wellness Incentive Program can change or end at any time. If you have additional questions, please contact the Benefits Team at: firstname.lastname@example.org. |Incomplete / ineligible form
||If the Wellness Screening Form was not filled out completely, was not signed by the employee or physician, or the information was illegible; ACS will not be able to process it. You will have to resubmit a completed form that is legible as soon as possible to avoid further delays in receiving the credit. To verify that your form(s) was received, please submit a question online by logging onto the Signature Wellness Website at www.yoursignaturewellness.com
|Form sent to wrong vendor
||Actuarial Consulting Services (ACS) is the vendor that manages the 2021 Wellness Incentive Program. The Wellness Screening Form must be sent to ACS online or by mail. To upload, visit www.yoursignaturewellness.com. Or, mail it to: Signature Aviation Wellness, P.O. Box 390841, Omaha, NE 68139.
||If your spouse is enrolled on your medical plan, they are also required to complete a biometric health screening and submit the Wellness Screening Form to ACS. The credit will not be awarded until both you and your spouse have earned your incentive. Credits are not applied retroactively.
|Invalid medical plan
||If you are enrolled in medical plans for Hawaii or San Francisco, you are not eligible to receive the credit. These locations are exempt from the Wellness Incentive Program.
|Invalid biometric dates
||Depending on what year you completed your biometric health screening, it will only roll over (1) year. If you earned your credit for lab work in 2019, it was good for all of 2020. You would be required to do the steps again in 2020, for it to rollover into the following year (2021).
